Educational Concern Forms for the 2025-2026 School Year
A great deal of time and consideration goes into creating classes each year. We consider each student's individual learning needs and strive to create
well-rounded classes that balance academics, social-emotional needs, and class size. While we encourage parents to submit requests for teachers
or friends to be together, we also ask you to share details about your child's unique educational needs. Due to the complexity of the placement process
and the possibility of changes in teacher assignments each year, we cannot guarantee specific teacher requests. However, all information shared will
be thoughtfully considered. Staffing for the 2025–2026 school year has not yet been finalized. However, we anticipate it will be very similar to this year's
staffing, with one notable change: Mr. Midget will teach 6th grade.
Forms are due Friday, May 23, 2025.
Conversation with or emailing your child's teacher or the principal will not be considered an official request.

2025-2026 Concern Form

The Bountiful Food Pantry’s Pantry Pack Program provides food for our most at-risk children over the weekend when they have an increased risk of going hungry. They work closely with the Davis School District and individual schools.
The Pantry provides Pantry Packs for more than 3,500 students every week throughout Davis School District.
The packs are quart-sized bags of child-friendly supplemental food provided to students every Friday. The pack includes two breakfasts, two lunches and several snacks.
At Canyon Creek, each grade has been assigned a specific item to help us make as many Pantry Packs as possible.
Thank you for supporting the Student Council with this project.
Please bring items to the lunchroom starting now until May 1st. Our goal is to have enough items for students to assemble 400 packs to donate.

Book Fair is Tuesday April 15 During Donuts with Grownups 7:30-8:50AM and 3:30-5:30 PM. It will also be open Wednesday and Thursday 3:30-5:30. In the gym.


Field Day Volunteers
We are looking for Field Day volunteers.
Field Day is Tuesday, May 20.
8:45 – 11:45 AM Session of Field Day - Kindergarten, 4th and 5th grade
12:15 – 3:15 PM Session of Field Day – 1st, 2nd, and 3rd grade
6th graders are split between the AM and PM sessions.

At Canyon Creek, safety is our number one priority. This year, the administration is working with Davis School District and Farmington Police to create a plan to increase safety inside and outside the building. It was suggested that the south gate closest to the portables and the field be locked. Students may have noticed in the morning that the gate is locked, and they will need to walk around the north side of the building to get to the back of the school. At the end of each day, for safety reasons, the gate will be unlocked to help filter out the rush of 750 students leaving at the same time. Because students arrive at school more gradually over 15-20 minutes, the gate can safely be locked in the morning.

Below is a refresher of some of Canyon Creek’s procedures and policies.
Students should not arrive at school before 8:20 a.m. because limited supervision is available. Parents are welcome to wait with their children on the playground but should not stand in a class line.
At 8:50 a.m. students will line up at their grade level doors and wait for teachers to invite their entry into the building.
Toys, including, but not limited to, fidgets, spinners, Pokeman or other trading cards, Legos, Barbies, or other dolls, and Kadamas, electronic games, walkie-talkies, cameras, iPods, etc., should not be brought to school or taken out to recess.
These items can disrupt school and/or contribute to the injury of students. Adult staff members may take these items and return them only to a parent or guardian. Bicycles, scooters, electric scooters, skateboard, rollerblades,
Heels may come to school, but NO wheeled vehicles should be ridden on the school grounds. These items must remain locked in the bike rack during the school day. The school is not responsible for any lost or stolen property.
Attendance -Regular and consistent attendance is essential for student growth and achievement. Under Utah law, it is the responsibility of parents or guardians to ensure that their children are punctual in attending school each school day.
Please call 402-0305 if your child is or will be absent. Parents will be notified of unexcused absences by an automated calling system which is activated each school day at 10 a.m.
Beginning in first grade, a student who misses 11 days or 10 consecutive days can be unenrolled at Canyon Creek and referred to Family and Students Services at the Davis School District.
Students can bring a phone or an electronic watch to school. However, phones should be turned off and in a BACKPACK during the school day.
If a teacher or staff member sees a student’s phone during the school day it will be confiscated, and only be released to an adult.

Dress Code:
Wear clothing that are appropriate for school... comfortable clothing that fits the weather and the activities you will be participating in.
Clothing should fit your body and not be extremely big or extremely small.
Underclothes should be fully covered.
You must wear shoes all day at school.
Words and images must be school appropriate (no curse words or images, no sexually explicit words)
No gang affiliated clothing will be allowed.
Entire area from armpit to armpit to bottom must be covered. No bellies should be showing. Shorts need to be 3-4 inches in length.
NO HATS.
NO Watches. (If your child needs to wear a watch, they should not be on it during class time, or without teacher permission)

In the corner of the vestibule by the office door is an orange table set up for parents to drop off student items.
Please use the sticky notes provided to write your child’s name and place it on the item. The office will make sure your student gets the item ASAP.
